PhilHealth Dependents

Who are the qualified dependents under PhilHealth?

1.  Legal spouse (not active PhilHealth member)
2.  Legitimate or illigitimate, legally adopted or step child below 21 years old, unmarried and unemployed
3.  Parents 60 years old above (not PhilHealth member or retiree). step parent
4.  Child above 21 but suffering from congenital disability, physical or mental or any disability that renders total dependent of the parent.

What is Benefits Payment Notice (BPN)?

Once your claim in PhilHealth has been approved and processed, PhilHealth will automatically send the copy of your Benefits Payment Notice or BPN to your registered address.  This is a one page notice indicating the members name, patients name, date of confinement, diagnosis, details of your hospital bills and philhealth payment including doctors payment.  This is to inform you on your benefits availed from PhilHealth.  Once you have received this, compare your statement of account and BPN.  Check at the lower portion of the BPN.  You can present this BPN to the hospital or doctor in case you are under-deducted or you have any reimbursement.

Self-Employed Contribution - PhilHealth

Self-employed or voluntary members of PhilHealth can pay their contribution amounting to P300 per quarter to any PhilHealth Office and any accredited banks or collecting agents.  They have the option to pay it semi-annual or annually.  The deadline for payment is the last working day of the quarter, i.e, March 31, June 30, September 30 and December 31 (if these days are not declared holiday or falls on Saturday or Sunday).

PhilHealth Claim Form I

Claim Form I is one of the forms needed in PhilHealth benefits availment.  This form can be secured from any PhilHealth accredited hospitals, employers or PhilHealth Offices nationwide.  It has two parts where Part I to be filled-up by members and Part II to be filled-up by employers (if employed).  This form has to be submitted to the billing of the hospital together with the supporting documents before the discharge of patient to avail of automatic PhilHealth deduction from your bill.  It is important that you supplied all the needed information so as not to delay the availment.
